1) The name of the cloned VM (we can place there a default string: ""$VMNAME-cloned"")
2) The label of the cloned VM

Alternatively, we can skip the dialog box and assume the name of the cloned VM will always be $VMNAME-cloned-$NUMBER, and make sure the number is such, that there is no conflict with an exisiting VM in the system.
